1/29/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Josiah cooperated with counselor during role play and communicated that he understands how his past behaviors have negatively affected him. Josiah was attentive and well behaved but mother reports at times when he cannot have his way, he will shut down with behaviors lasting about 2-4 minutes in duration. Behavior: Josiah was observed displaying off-task behaviors when discussing his behaviors with his mother as evidenced by him screaming with behaviors lasting up to 2 minutes in duration or when his mother corrected present behaviors. Intervention: QMHP-C was able to get each family member to communicate with a clam voice tone, by issuing rules before talking. QMHP-C reminded the family that the rules consist of allowing each family member to speak without interruption. QMHP-C assisted Josiah how to identify more effective ways to improve self-esteem. QMHP-C assisted the parent on how to specifically define and identify problem behaviors, identify their reactions to the behavior, determine whether the reaction encourages or discourages the behaviors, and generate alternatives to the problem behavior. Intervention: QMHP-C assisted the parent on how to implement key parenting practices consistently, including establishing realistic age appropriate rules for acceptable and unacceptable behavior, prompting of positive reinforcement to encourage more positive behavior. QMHP-C observed Josiah in the home to determine if he required assistance. QMHP-C observed Josiah complying with home rules and expectations. QMHP-C talked to Josiah about his confidence levels and how they can affect his thoughts and behaviors. QMHP-C encouraged Josiah to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP-C reminded Josiah of his strengths and abilities. QMHP-C praised Josiah for his efforts and being respectful to his mom and sibling. Response: Each family member understood the expectations and family rules for family session. Each family member verbalized how they were feeling and how their day was going. Mother verbalized issues and concerns pertaining to the home. Mother verbalized Josiah behaviors in the home. Each family member understood that the horse playing in the home needs to decrease for safety reasons especially with mom being pregnant. Each family member understood that their chores need to be completed or consequences for behaviors would apply. Josiah changed his attitude and participated in family session. Response: Each family participated and cooperated in family activity. Josiah utilized conflict management skills at home, school, and in the community. Josiah exhibited techniques for appropriate self-expression and active listening. Josiah expressed a belief in self as being lovable and capable. Josiah demonstrated appropriate interpersonal skills, assertiveness, confidence in self and ability to problem-solve on his own. Josiah communicated positive self-talk and affirmations. Josiah was able to recognize personal strengths and abilities to perform better in school. Josiah was observed complying to authority requests and demands the first time they are given.
